Please ensure you print out both the Registration Form AND the Medical Form.  Medical forms must be signed by your child's doctor after Jan 1, 2022 AND it must be returned to DAYO before August 1.  Your child cannot participate without this form.  The blue school medical forms are now accepted by Pop Warner.
